Moving on Team

The Moving on Team, offers support to young care leavers across East Anglia.

Funded exclusively by Break's charitable income, The Moving on Team consists of Transition and Mentoring Services. We support and encourage young people to achieve their full potential, without letting their past experiences limit their outcomes or ambitions.

Transition Workers offer advice and support to young people aged 16 and over, including those with a disability, living in Break's homes and in the community. We help with choices in their education, careers and accommodation and offer practical support with the transition into adulthood. Our support continues for as long as it is needed - our young people can always turn to us, regardless of their age.


Our volunteer Mentors give their time to develop a non-judgemental and trusting relationship with a child or young person to help them make positive changes. We recruit Mentors from across East Anglia who have a few hours to spare every couple of weeks over a sustained period of time. Mentors support young people in a variety of settings including: Children in Care, Care Leavers, Therapeutic Foster Placements, Young Parents, Children with Disabilities and Young Carers.
